HOW TO ENTER: Beginning 12:00 am MT on January 24, 2022 through 11:59 pm MT February 24, 2022, visit https://survey.alchemer.com/s3/6705439/BioTRUST-New-Product-Survey-JAN22 or https://survey.alchemer.com/s3/6710118/BioTRUST-New-Product-Survey-January-2022, complete the required information and submit as directed.

All entries must be received no later than 11:59 pm MT February 24, 2022 to be eligible. By entering, you certify that you are at least 18 years of age or older at the time of entry.

ELIGIBILITY: This contest is open only to legal residents of the United States who are 18 years of age or older on the date of the contest entry submission. Employees, directors and officers and their immediate families (spouses and siblings, parents and children and their spouses) and household members of the Sponsor and its respective parent companies, subsidiaries and affiliates, distributors, dealerships and companies involved in the implementation and execution of the contest are not eligible. All federal, state and local laws and regulations apply. Void where prohibited by law, rule or regulation.

By participating, you agree to accept these Official Rules and the decisions of the Sponsor, whose decisions are final and binding in all respects. By entering this contest, you also agree to release, discharge and hold harmless Sponsor and their respective affiliates, subsidiaries, advertising, promotion and internet agencies, and contractors, and their respective officers, directors, employees and agents, from any and all damages whether direct or indirect, which may be due to or arise out of participation in the contest or the acceptance, use/misuse or possession of a prize.

INTERNET ENTRY: Sponsor is not responsible for incomplete, lost, late, damaged, illegible or misdirected entries, or for any technical problems, malfunctions of any telephone lines, computer systems, servers, providers, hardware/software, lost or unavailable network connections or failed, incomplete, garbled or delayed computer transmission or any combination thereof. Sponsor is not responsible for any liability for damage to any computer system resulting from participation in or accessing or downloading information in connection with this contest.

CAUTION: ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS CONTEST IS A VIOLATION OF CRIMINAL AND CIVIL LAW. SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH INDIVIDUAL TO THE FULLEST EXTENT PERMITTED BY LAW.

Sponsor reserves the right at its sole discretion to disqualify any individual who tampers with the entry process. Sponsor assumes no responsibility for undeliverable emails resulting from any form of active or passive email filtering by a user's Internet Service Provider and/or email client or for insufficient space in user's email account to receive email. Sponsor reserves the right, in its sole discretion, to cancel or suspend this contest should causes beyond the control of Sponsor corrupt the administration, security or proper operation of the contest. Sponsor reserves the right to modify the contest or disqualify entrants if fraud, misconduct or technical failures destroy the integrity of the contest, as determined by Sponsor, in its sole discretion.

GENERAL RULES: Winners will be announced and notified by email on or about March 15, 2022. Failure to timely respond may result in an alternate winner being selected. No substitution or transfer of prize permitted. Sponsor reserves the right to substitute prize of equal or greater value or to substitute the cash equivalent of the prize at its sole discretion. All federal, state and local taxes are the sole responsibility of winner. Winner will be issued an IRS 1099 form for the retail value of the prize. Acceptance of prize constitutes permission to Sponsor and its agencies to use winner's image; as well as their name, likeness, and submission, in any form, in whole or in part, for purposes of public relations, promotional advertising and trade purposes in any and all media, without territorial, time, use or other limitation without further compensation, unless prohibited by law. All material submitted becomes the sole property of Sponsor and will not be returned.

Sponsor is not responsible for any typographical or other error in the printing of the offer, administration of the contest or in the announcement of the prize. All winners must certify that they will not seek reimbursement for any products received as a prize. Sponsor does not make any warranty, representation or guarantee, express or implied, in fact or in law, relative to the use of any prize, including, without limitation, quality, merchantability and fitness for a particular purpose.

NOTWITHSTANDING ANY OTHER PROVISION OF THIS AGREEMENT TO THE CONTRARY, SPONSOR SHALL NOT BE LIABLE TO ENTRANT OR ANY THIRD PARTY FOR ANY INDIRECT, CONSEQUENTIAL, SPECIAL, PUNITIVE, EXEMPLARY OR INCIDENTAL DAMAGES (INCLUDING DAMAGES FOR LOST PROFITS), EVEN IF SPONSOR HAS BEEN MADE AWARE OF THE POSSIBILITY OF SUCH DAMAGES. SOME JURISDICTIONS DO NOT ALLOW THE LIMITATIONS OR EXCLUSION OF LIABILITY FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES, SO THE ABOVE MAY NOT APPLY TO YOU. Sponsor reserves the right to terminate, suspend, withdraw or amend the contest for any reason.

PRIZES & APPROXIMATE RETAIL VALUES ("ARV"): Grand Prize: Five (5) grand prize winners will be selected at random by Sponsor.  Five (5) grand prize winners, subject to eligibility and compliance with these Official Rules, will receive $250 worth of BioTRUST Products of their choice. (ARV: $250 each based on the single unit retail price of the product as currently listed on www.biotrust.com)

Winner is solely responsible for all local, state or federal taxes. Please allow two (2) to four (4) weeks for delivery of prizes. Prizes returned as undeliverable will not be reshipped and will be considered fulfilled and finalized for the purpose of awarding the prized in this Giveaway.

SELECTION OF WINNERS: The Grand Prize winners will be selected in a random drawing conducted from among all eligible entries received.

Winner will be contacted via email. Odds of winning a prize will depend upon the number of eligible entries received. The potential winner may be required to complete an affidavit of eligibility/release of liability within five (5) business days of attempted delivery of same. Affidavit must certify entrant's eligibility, that entrant has not won a previous prize or award and that the entrant has followed the rules of the contest. Failure to timely comply with all requirements, including the 5-day time period, or return of any prize notification as undeliverable will result in disqualification and an alternate winner will be selected.

Verification of Potential Winners:  ALL POTENTIAL WINNERS ARE SUBJECT TO VERIFICATION BY SPONSOR WHOSE DECISIONS ARE FINAL AND BINDING IN ALL MATTERS RELATED TO THE PROMOTION.  AN ENTRANT IS NOT A WINNER OF ANY PRIZE UNLESS AND UNTIL ENTRANT'S ELIGIBILITY HAS BEEN VERIFIED AND ENTRANT HAS BEEN NOTIFIED THAT VERIFICATION IS COMPLETE.

CHOICE OF LAW: Under no circumstances will entrants be permitted to obtain awards for, and entrant hereby waives all rights to claim, punitive, incidental or consequential damages, or any other damages, including attorneys' fees, other than entrant's actual out-of-pocket expenses (i.e., costs associated with entering this contest), and entrant further waives all rights to have damages multiplied or increased. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of entrants and Sponsor in connection with the contest, shall be governed by, and construed in accordance with, the substantive laws of the State of Texas, USA without regard to Texas choice of law rules. By completing the entry form, participant submits to the personal jurisdiction of the State of Texas.
